How to Obtain a National Provider Identification (NPI) Number

The federal government and many private health care organizations track health care providers using an NPI number. The number is “information free”; it is not made up of any of the provider’s personal identifiable information. This information is maintained by the federal government and is not released to the public. However, clinicians’ names and provider locations are public through the system.

To obtain an NPI, go to: https://nppes.cms.hhs.gov/#/ and follow the instructions on the screen. OT Pro Tips:- – Make sure you apply for an individual NPI (as opposed to a group) – When asked for business address and a practice location, you may use your home address – When asked to supply a contact person, supply your own information – You will not be issued an NPI immediately. It may take up to 20 business days for the NPI number to be issued. If you are having problems using the system, contact 1-800- 465- 3203 (NPI Toll-free)
